How can I carry my rubbish story on?

Now you'e set it all up, it's time to outline the purpose. Is the doctor trying to affect the war or simply survive it? If he's trying to affect it, is he fighting it or trying to end it? If so, introduce more characters that have a similar aim and create bonds between the doctor and them. If he's simply trying to survive it, have him change somehow, like moving or leaving his home, or else give him an early plot device (such as war affecting an area or hurting someone he loves) to make him take a step forward and change his path.
